Agreement Conditions*

I understand and agree that my completion and submission of this form to John Nelson, a notary public, serves as a valid contract between me and the notary public, after accepted by the notary public, and remains valid unless cancelled by text and/or email (949-614-5291, jcnelson.notary@gmail.com) a minimum of 2 hours before the scheduled appointment. ALL APPOINTMENTS REQUIRE A $150 GOOD FAITH DEPOSIT. This deposit will be subtracted from the full fee for services rendered. This deposit will be refunded for appointments that are cancelled earlier than 2 hours before the appointment time.
